I used to make my own, they are easy really but I now use quickbooks as an accounts package. You can design the template to your own requirements and have more than 1 invoice type if you want.
Very cheap package, I was recommended it by a friend who owns a large I.T company

I use a cheap carbon copy invoice book (with a stamp) for smaller jobs and print out on Word for bigger ones.

I too just print out on Word using my own design template. Can't be bothered learning a new accounts package just use a simple excel spreadsheet to keep the tax man happy!

I initially created my own but I wanted something slightly more professional so I used a free package from the Internet called "express invoice" - it allows you to do pretty much everything like customer databases upload your own logo and create accounts reports etc. I've got it for the iMac so not sure you can get it for pc but I would expect you can.
I use the customer database a lot now as I've built it up you can easily access your customers details with a click of a button
